Indications
- Control of severe allergic conditions in asthma, atopic dermatitis, contact dermatitis, etc.
- Treatment of collagen diseases such as lupus erythematosus, rheumatoid arthritis, etc.
- Management of various dermatologic diseases and endocrine disorders
- Treatment of neoplastic diseases, nervous system conditions, ophthalmic diseases, etc.
Pharmacology
- Synthetic glucocorticoid
- Decreases inflammation by inhibiting leukocyte migration
- Suppresses normal immune response
Dosage & Administration
- Oral dosage ranging from 1mg to 10mg for adults
- Higher dosages may be required temporarily in some patients
- Intravenous, intramuscular, or local injection administration
- Dosage varies based on severity of condition and patient response
Interactions
- Enhanced potassium loss with diuretics and/or cardiac glycosides
- Impaired glucose tolerance with antidiabetics
- Increase in gastrointestinal ulceration with non-steroidal anti-inflammatory drugs
- Alteration in need for oral anticoagulants
Contraindications
- Gastric and duodenal ulcers
- Systemic and ophthalmic fungal infections
- Viral infections, e.g. varicella and herpes genitalis infections
- Glaucoma
- Hypersensitivity to corticosteroids
Side Effects
- Cushing-like syndrome
- Fluid and electrolyte disturbances
- Musculo-skeletal effects including myopathy, osteoporosis
- Gastro-intestinal effects such as ulceration and haemorrhage
- Dermatological effects like skin atrophy, striae, etc.
Pregnancy & Lactation
- Pregnancy Category C
- Use only if potential benefit justifies potential risk to fetus
- Appear in breast milk, advised not to breast-feed at high dosages
Precautions & Warnings
- Use lowest possible dose
- Caution in patients with congestive heart failure, hypertension, or renal insufficiency
- Minimize drug-induced secondary adrenocortical insufficiency by gradual reduction of dosage
Use in Special Populations
- Titrate pediatric patients to lowest effective dose
Overdose Effects
- Treatment by supportive and symptomatic therapy
