Pharmacology
Synthetic glucocorticoid decreasing inflammation by inhibiting leukocyte migration and reversing increased capillary permeability. Suppresses normal immune response.
Dosage & Administration
- Tablet Dosage:
- Daily oral dosages vary from 1 mg to 10 mg for adults
- Children's dosage varies from 0.03-0.20 mg/kg body weight
- Injection Administration:
- Intravenous (IV), intramuscular (IM), or local injection
- Intravenous injections of massive doses given slowly over several minutes
- Intra-articular injections given under strictly aseptic conditions
Interactions
- Diuretics and/or cardiac glycosides
- Antidiabetics
- Non-steroidal anti-inflammatory drugs
- Oral anticoagulants
- Liver enzyme inducing drugs
- Salicylates
- Antacids
Contraindications
- Gastric and duodenal ulcers
- Systemic and ophthalmic fungal infections
- Viral infections
- Glaucoma
- Hypersensitivity to corticosteroids
Side Effects
- Endocrine and Metabolic Disturbances: Cushing-like syndrome, hirsutism, menstrual irregularities, premature epiphyseal closure
- Fluid and Electrolyte Disturbances: Sodium and fluid retention, potassium loss, hypokalaemic alkalosis
- Musculo-skeletal Effects: Myopathy, abdominal distension, osteoporosis
- Gastro-intestinal Effects: Gastric and duodenal ulceration, perforation, haemorrhage
- Dermatological Effects: Impaired wound healing, skin atrophy, striae, increased sweating, acne
- Central Nervous System Effects: Psychic disturbances, convulsions, pseudotumor cerebri in children
- Ophthalmic Effects: Glaucoma, increased intraocular pressure, posterior subcapsular cataracts
- Immunosuppressive Effects: Increased susceptibility to infections, decreased responsiveness to vaccination and skin tests
Pregnancy & Lactation
Category C - Use only if potential benefit justifies potential risk to fetus. Glucocorticoids appear in breast milk, caution advised for breastfeeding mothers.
Precautions & Warnings
- General: Use lowest possible dose, reduce dosage gradually
- Cardio-renal: Caution in patients with heart failure, hypertension, or renal insufficiency
- Endocrine: Minimize drug-induced secondary adrenocortical insufficiency by gradual reduction of dosage
- Gastrointestinal: Use with caution in peptic ulcers, diverticulitis, and ulcerative colitis
- Musculoskeletal: Consider osteoporosis risk before initiation of therapy
- Neuro-psychiatric: Watch for acute myopathy with high doses, monitor for psychic disturbances
Use in Special Populations
Pediatric patients should be titrated to the lowest effective dose to minimize potential growth effects
Overdose Effects
Treatment is by supportive and symptomatic therapy
