Pharmacology
Hydrocortisone inhibits prostaglandin synthesis by inhibiting the enzyme phospholipase A2. It reduces various vasoactive agents released during inflammation and controls the rate of protein synthesis.
Dosage & Administration
- Initial dosage of 20 mg to 240 mg of hydrocortisone per day
- Variable dosage requirements
- Individualized dosage based on disease and patient response
- Maintenance dosage determination
- Constant monitoring needed
- Gradual withdrawal recommended for long-term therapy
- Adult injection dosage range: 100 mg to 500 mg
- Children injection dosage: 25 mg (up to 1 year), 50 mg (1-5 years), 100 mg (6-12 years)
Interaction
Drugs that induce hepatic enzymes may increase the clearance of corticosteroids. Drugs such as Troleandomycin and Ketoconazole may inhibit the metabolism of corticosteroids and decrease their clearance. Corticosteroid dose should be titrated to avoid steroid toxicity.
Contraindications
Contraindicated in severe systemic fungal infections and patients with known hypersensitivity to any component of this product
