Indications

  • Treatment of anovulation or oligo-ovulation in females
  • Infertility treatment for males with oligospermia and poor sperm motility

Pharmacology

  • Clomiphene has antiestrogenic effects
  • Stimulates ovulation by competing with estrogen for binding sites
  • Increases secretion of follicle-stimulating hormone (FSH) and luteinizing hormone (LH)

Dosage & Administration

  • For females: 50 mg daily for 5 days, starting from the second to fifth day of the menstrual cycle
  • Dosage may be adjusted for women with polycystic ovary syndrome
  • For males: 25 mg once daily up to 90 days, with a recommended break pattern
  • Different dosage for patients undergoing haemodialysis and the elderly
  • Ovulation usually occurs 4 to 10 days after the last day of treatment

  • No reported drug interactions

Contraindications

  • Pregnancy
  • Liver insufficiency
  • Abnormal vaginal bleeding (uterine tumor, ovarian cyst)

Side Effects

  • Adverse effects are rare at recommended dosage
  • Incidence and severity related to dose and duration of treatment
  • Reversible after discontinuation
  • Visual disturbances, ovarian hyperstimulation, hot flushes, abdominal discomfort, nausea, vomiting, depression, insomnia, breast tenderness, headache, spotting, menorrhagia, endometriosis, convulsions, weight gain, rashes, dizziness, hair loss reported
  • Doses over 100 mg/day for five days associated with higher incidence of side effects
  • Rare reports of ovarian cancer with fertility drugs

Pregnancy & Lactation

  • Not recommended in pregnancy
  • May reduce lactation if administered during lactation period

Precautions & Warnings

  • Only administered under medical supervision
  • Complete pelvic examination essential before treatment
  • Incidence of multiple pregnancies increased during clomiphene therapy
