Indications

• Treatment of men with erectile dysfunction
• Treatment of pulmonary arterial hypertension

Description

• Selective, reversible inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5)
• Causes smooth muscle relaxation and inflow of blood into the penile tissues

Pharmacology

• Mechanism of Action: involves release of nitric oxide and increased levels of cyclic guanosine monophosphate (cGMP)
• Rapidly absorbed after oral administration with a mean absolute bioavailability of 41%

Dosage & Administration

• Recommended dose for erectile dysfunction is 50 mg taken approximately 1 hour before sexual activity
• Recommended dose for pulmonary arterial hypertension is 20 mg three times a day

Interaction

• Metabolism mediated by the cytochrome P450 (CYP) isoforms 3A4 and 2C9
• Inhibitors of CYP3A4 may increase Adegra clearance

Contraindications

• Contraindicated in patients with hypersensitivity to any component of the medication
• Contraindicated in patients using organic nitrates

Side Effects

• Can cause body edema, migraine, syncope, abnormal dreams, and prolonged erection
• Reported cardiovascular, cerebrovascular, and vascular events post-marketing

Pregnancy & Lactation

• Pregnancy category B with no adequate and well-controlled studies in pregnant women
• Not indicated for use by women

Precautions & Warnings

• Caution advised when co-administered with alpha-blockers
• Potential for cardiac risk of sexual activity in patients with preexisting cardiovascular disease

Overdose Effects

• In cases of overdose, standard supportive measures should be adopted as required
