Indications
- Treatment of men with erectile dysfunction
- Treatment of pulmonary arterial hypertension
Description
- Selective, reversible inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5)
- Causes smooth muscle relaxation
- Increases inflow of blood into penile tissues
- Requires sexual stimulation for effectiveness
Pharmacology
- Mechanism of Action involves release of nitric oxide (NO) in the corpus cavernosum
- Activation of guanylate cyclase leading to increased cGMP levels
- Rapid absorption after oral administration
- Metabolized predominantly by hepatic metabolism
- Cleared by hepatic microsomal isoenzymes
- Reduced clearance in elderly volunteers
Dosage & Administration
- Recommended dose for erectile dysfunction is 50 mg
- Can be taken 1 hour before sexual activity
- Dose may be increased to a maximum of 100 mg or decreased to 25 mg
- Maximum recommended dosing frequency is once per day
- Recommended dose for pulmonary arterial hypertension is 20 mg three times a day
Interaction
- Metabolism mediated by cytochrome P450 (CYP) isoforms 3A4 and 2C9
- Inhibitors of these isoenzymes may reduce clearance
- Stronger CYP3A4 inhibitors expected to have greater effects
Contraindications
- Hypersensitivity to any component of the medication
- Use with organic nitrates

- Includes body, cardiovascular, digestive, hemic and lymphatic, metabolic and nutritional, musculoskeletal, nervous, respiratory, skin and appendages, special senses, and urogenital effects
Pregnancy & Lactation
- Pregnancy category B
- Not indicated for use by women
Precautions & Warnings
- Evaluation of erectile dysfunction should include determination of potential underlying causes
- Caution advised when co-administered with alpha-blockers
- Patients with preexisting cardiovascular disease should use with caution
- Potential for cardiac risk of sexual activity in patients with preexisting cardiovascular disease
- Particular sensitivity in patients with certain underlying conditions
Overdose Effects
- Adverse events similar to those seen at lower doses
- Standard supportive measures should be adopted in case of overdose
- Renal dialysis not expected to accelerate clearance
