Indications
- Treatment of men with erectile dysfunction
- Indication in pulmonary arterial hypertension
Description
- Selective, reversible inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5)
- Produces smooth muscle relaxation and inflow of blood into penile tissues
Mode of Action
- Involves release of nitric oxide during sexual stimulation
- Enhances effect of nitric oxide by inhibiting phosphodiesterase type 5 (PDE5)
Pharmacology
- Rapid absorption after oral administration
- Mean absolute bioavailability of 41%
- Eliminated predominantly by hepatic metabolism
- Reached maximum plasma concentrations within 30 to 120 minutes of oral dosing
Dosage & Administration
- Recommended dose of 50 mg for most patients
- Maximum recommended dosing frequency is once per day
Interaction
- Metabolism mediated by cytochrome P450 isoforms 3A4 and 2C9
- Inhibitors of these isoenzymes may reduce clearance
Contraindications
- Hypersensitivity to any component of the medication
- Contraindicated in patients using organic nitrates

- Body as a whole: face edema, photosensitivity reaction, shock, asthenia, pain, chills
- Cardiovascular: angina pectoris, AV block, migraine, syncope, tachycardia
- Digestive: vomiting, glossitis, colitis, dysphagia, gastritis, gastroenteritis
- Hemic and Lymphatic: anemia and leukopenia
- Metabolic and Nutritional: thirst, edema, gout, unstable diabetes, hyperglycemia
- Musculoskeletal: arthritis, arthrosis, myalgia, tendon rupture, tenosynovitis
- Nervous: ataxia, hypertonia, neuralgia, neuropathy, paresthesia, tremor
Pregnancy & Lactation
- Pregnancy category B
- No adequate and well-controlled studies in pregnant women
Precautions & Warnings
- Caution advised when co-administered with alpha-blockers
- Potential for cardiac risk of sexual activity in patients with preexisting cardiovascular disease
Overdose Effects
- Adverse events similar to those seen at lower doses
- Renal dialysis not expected to accelerate clearance
