Indications
- Treatment of men with erectile dysfunction
- Treatment of pulmonary arterial hypertension
Description
- Selective, reversible inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5)
- Causes smooth muscle relaxation
- Increases inflow of blood into penile tissues
- No effect in the absence of sexual stimulation
Pharmacology
- Mechanism of Action involves release of nitric oxide (NO) in the corpus cavernosum
- Activation of the enzyme guanylate cyclase
- Increase in levels of cyclic guanosine monophosphate (cGMP)
- Smooth muscle relaxation in the corpus cavernosum
Dosage & Administration
- Recommended dose for erectile dysfunction: 50 mg, taken approximately 1 hour before sexual activity
- Can be taken anywhere from 4 hours to 0.5 hour before sexual activity
- Recommended dose for pulmonary arterial hypertension: 20 mg three times a day
Interaction
- Metabolism mediated by cytochrome P450 (CYP) isoforms 3A4 and 2C9
- Inhibitors of these isoenzymes may reduce clearance
- Inducers of these isoenzymes may increase clearance
Contraindications
- Hypersensitivity to any component of the medication
- Concomitant use with organic nitrates
Side Effects
- Includes body-related, cardiovascular, digestive, hemic and lymphatic, metabolic and nutritional, musculoskeletal, nervous, respiratory, skin and appendages, special senses, and urogenital effects
Pregnancy & Lactation
- Pregnancy category B
- Not indicated for use by women
- No evidence of teratogenicity or embryotoxicity in animal studies
Precautions & Warnings
- Caution advised when co-administered with alpha-blockers
- May lower blood pressure significantly
- Considerations for patients with preexisting cardiovascular disease
- No controlled clinical data for certain patient groups
