Indications

Description
- Selective, reversible inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5)
- Increases levels of cGMP in corpus cavernosum
- Results in smooth muscle relaxation and inflow of blood into penile tissues
Pharmacology
- Mechanism of Action: Involves release of nitric oxide in corpus cavernosum
- Enhances effect of nitric oxide by inhibiting PDE5
- Rapid absorption after oral administration
- Mean absolute bioavailability of 41%
- Eliminated predominantly by hepatic metabolism
Dosage & Administration
- Recommended dose of 50 mg for erectile dysfunction, may be adjusted based on toleration
- Maximum recommended dosing frequency is once per day
- Recommended dose of 20 mg three times a day for pulmonary arterial hypertension
Interaction
- Metabolism mediated by cytochrome P450 isoforms 3A4 and 2C9
- Effects of inhibitors and inducers on Fulfeel clearance
- Possible increase in Fulfeel levels with co-administration of CYP3A4 inhibitors
- No effect on ritonavir pharmacokinetics
Contraindications
- Hypersensitivity to any component of the medication
- Use of organic nitrates, either regularly or intermittently

Precautions & Warnings
- Caution advised when PDE5 inhibitors are co-administered with alpha-blockers
- Consideration for patients with underlying conditions
- Potential for cardiac risk of sexual activity in patients with preexisting cardiovascular disease
- Sensitivity of certain groups to vasodilators including Fulfeel
- Prolonged erection and priapism reported infrequently
Overdose Effects
- Adverse events similar to lower doses but increased in incidence rates and severities
- Standard supportive measures recommended in cases of overdose
