Indications

Description
- Selective inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5)
- Produces increased levels of cGMP in the corpus cavernosum through inhibition of PDE5
- Results in smooth muscle relaxation and inflow of blood into penile tissues
Mechanism of Action
- Release of nitric oxide in the corpus cavernosum during sexual stimulation
- Activation of the enzyme guanylate cyclase
- Increased levels of cyclic guanosine monophosphate (cGMP)
- Smooth muscle relaxation in the corpus cavernosum
- Inflow of blood into the penile tissues
Pharmacokinetics and Metabolism
- Rapid absorption after oral administration
- Mean absolute bioavailability of 41%
- Elimination predominantly by hepatic metabolism
- Converted to an active metabolite with similar properties to the parent compound
- Terminal half lives of about 4 hours
Dosage & Administration
- Recommended initial dose of 50 mg taken 1 hour before sexual activity
- May be taken anywhere from 4 hours to 0.5 hour before sexual activity
- Maximum recommended dose of 100 mg
- Maximum recommended dosing frequency is once per day
Interactions
- Metabolism mediated by cytochrome P450 (CYP) isoforms 3A4 and 2C9
- Potential for reduced clearance with inhibitors of these isoenzymes
Contraindications
- Hypersensitivity to any component of the medication
- Concomitant use with organic nitrates

Precautions & Warnings
- Caution advised when co-administered with alpha-blockers
- Consideration needed for patients with underlying cardiovascular disease or bleeding disorders
- Recommended with caution for patients with anatomical deformation of the penis or conditions predisposing to priapism
- No controlled clinical data on safety or efficacy in certain patient groups
