Indications
- Treatment of men with erectile dysfunction
- Treatment of pulmonary arterial hypertension
Description
- Selective inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5)
Pharmacology
- Mechanism of action involves release of nitric oxide (NO) during sexual stimulation
- Inhibition of PDE5 increases levels of cGMP, leading to smooth muscle relaxation and inflow of blood
- Rapid absorption after oral administration
- Predominantly hepatic metabolism
- Maximum observed plasma concentrations are reached within 30 to 120 minutes of oral dosing
- Metabolized by hepatic microsomal isoenzymes
- Effects on elderly volunteers
Dosage & Administration
- Recommended dose for erectile dysfunction is 50 mg taken approximately 1 hour before sexual activity
- Dose may be increased to a maximum of 100 mg or decreased to 25 mg based on effectiveness and toleration
- Recommended dose for pulmonary arterial hypertension is 20 mg three times a day
Interaction
- Metabolism mediated by cytochrome P450 (CYP) isoforms 3A4 and 2C9
- Inhibitors of these isoenzymes may reduce drug clearance
- Inducers of these isoenzymes may increase drug clearance
- Cautions with coadministration of other drugs affecting CYP3A4
Contraindications
- Hypersensitivity to any component
- Concurrent use of organic nitrates
