Indications
- Treatment of erectile dysfunction in men
- Indicated in pulmonary arterial hypertension
- Requires sexual stimulation for effectiveness
Description
- Selective, reversible inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5)
- Results in smooth muscle relaxation and inflow of blood into the penile tissues
- Has no effect in the absence of sexual stimulation
Pharmacology
- Mechanism of Action involves release of nitric oxide (NO) during sexual stimulation
- Inhibition of PDE5 by Niagra increases levels of cGMP, producing smooth muscle relaxation and inflow of blood
- Rapidly absorbed after oral administration
Dosage & Administration
- Recommended dose for erectile dysfunction is 50 mg, to be taken approximately 1 hour before sexual activity
- Can be increased to a maximum recommended dose of 100 mg or decreased to 25 mg based on effectiveness and toleration
- Recommended dose for pulmonary arterial hypertension is 20 mg three times a day, approximately 4-6 hours apart
Interactions
- Metabolism mediated by CYP isoforms 3A4 and 2C9
- Inhibitors of these isoenzymes may reduce Niagra clearance, while inducers may increase Niagra clearance
Contraindications
- Contraindicated in patients with hypersensitivity to any component of the medication
- Contraindicated in patients using organic nitrates
Side Effects
- Includes cardiovascular, digestive, hemic and lymphatic, metabolic and nutritional, musculoskeletal, nervous, respiratory, and urogenital effects
- Includes serious cardiovascular, cerebrovascular, and vascular events
Pregnancy & Lactation
- Pregnancy category B, not indicated for use in women
- No evidence of teratogenicity or embryotoxicity in animal studies
Precautions & Warnings
- Caution advised when co-administered with alpha-blockers
- Consideration should be given to patients with underlying cardiovascular disease
- Particular sensitivity in patients with left ventricular outflow obstruction or severely impaired autonomic control of blood pressure
Overdose Effects
- Adverse events similar to lower doses, but with increased incidence rates and severities
