Indications
- Treatment of erectile dysfunction in men
- Treatment of pulmonary arterial hypertension
Mode of Action
- Inhibition of phosphodiesterase type 5 (PDE5)
- Increased levels of cyclic guanosine monophosphate (cGMP)
- Smooth muscle relaxation
- Increased blood flow to penile tissues
Pharmacology
- Mechanism of Action: Involves the release of nitric oxide and activation of guanylate cyclase
- Pharmacokinetics and Metabolism: Rapid absorption, hepatic metabolism, active metabolite with similar properties, terminal half-life of about 4 hours
- Absorption and Distribution: Rapid absorption with maximum plasma concentrations reached within 30 to 120 minutes
- Metabolism and Excretion: Clearance by hepatic microsomal isoenzymes, excreted primarily in feces
Dosage & Administration
- Erectile Dysfunction:
- Recommended dose of 50 mg taken approximately 1 hour before sexual activity
- Max dose of 100 mg or min dose of 25 mg
- Recommended dosing frequency is once per day
- Pulmonary Arterial Hypertension: Recommended dose of 20 mg three times a day
Interaction
- Metabolism mediated by cytochrome P450 isoforms 3A4 and 2C9
- Potential effects of CYP3A4 inhibitors and inducers on clearance
Contraindications
- Hypersensitivity to any component of the medication
- Concomitant use with organic nitrates
Side Effects
- Includes body reactions, cardiovascular effects, digestive issues, hemic and lymphatic reactions, metabolic and nutritional reactions, musculoskeletal effects, nervous system effects, respiratory effects, skin and appendage reactions, special senses reactions, urogenital effects, and serious cardiovascular and cerebrovascular events

- Animal studies show no evidence of teratogenicity or embryotoxicity
Precautions & Warnings
- General: Evaluation of underlying causes of erectile dysfunction, caution with alpha-blocker co-administration
- Cardiac Risk: Consideration of cardiac risk related to underlying cardiovascular status
- Sensitivity: Patients with specific underlying conditions may be sensitive to vasodilators
- Specific Groups: No controlled clinical data on safety in certain patient groups
