Description
- Selective, reversible inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5)
Mode of Action
- Inhibition of PDE5 by Silfil increases levels of cGMP in the corpus cavernosum, resulting in smooth muscle relaxation and inflow of blood into the penile tissues
- Rapid absorption after oral administration with mean absolute bioavailability of 41%
- Elimination predominantly by hepatic metabolism
- Metabolized by cytochrome P450 3A4 and converted to an active metabolite with similar properties to the parent compound
Dosage & Administration
- Recommended dose for erectile dysfunction is 50 mg taken approximately 1 hour before sexual activity. Can be increased to a maximum of 100 mg or decreased to 25 mg based on effectiveness and toleration
- Recommended dose for pulmonary arterial hypertension is 20 mg three times a day
Interaction
- Metabolism is mediated by the cytochrome P450 (CYP) isoforms 3A4 and 2C9, and inhibitors/inducers of these isoenzymes may affect Silfil clearance
- Co-administration with CYP3A4 inhibitors may increase Silfil plasma levels
- Co-administration with alpha-blockers should be done cautiously
- Use with nitrates is contraindicated
- Potential effects with other medications such as antihypertensives and protease inhibitors

- Hypersensitivity to any component of the medication
- Use with nitrates
Side Effects
- Various body system-related side effects including cardiological, digestive, hemic and lymphatic, metabolic and nutritional, musculoskeletal, nervous, respiratory, dermatological, and urogenital effects
- Serious cardiovascular, cerebrovascular, and vascular events reported post-marketing
Pregnancy & Lactation
- Pregnancy category B with no adequate and well-controlled studies in pregnant women
Precautions & Warnings
- Caution advised when co-administering with alpha-blockers
- Considerations for patients with specific conditions such as cardiovascular disease, bleeding disorders, and anatomical deformation of the penis
- Potential for cardiac risk of sexual activity in patients with preexisting cardiovascular disease
- Sensitivity of vasodilators in patients with certain conditions
- Warnings for prolonged erection and priapism, and precautions for patients taking ritonavir
Overdose Effects
- Adverse events similar to those seen at lower doses but with increased incidence rates and severities
- Standard supportive measures should be adopted in cases of overdose
- Renal dialysis not expected to accelerate clearance as Silfil is highly bound to plasma proteins
