Indications
- Treatment of men with erectile dysfunction
- Indicated in pulmonary arterial hypertension
Description
- Selective, reversible inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5)
- Results in smooth muscle relaxation and inflow of blood into the penile tissues, thereby producing an erection
Pharmacology
- Mechanism of Action: Release of nitric oxide in the corpus cavernosum during sexual stimulation, resulting in increased levels of cyclic guanosine monophosphate (cGMP)
- Pharmacokinetics and Metabolism: Rapid absorption after oral administration, hepatic metabolism, converted to an active metabolite with properties similar to the parent
- Absorption and Distribution: Maximum observed plasma concentrations are reached within 30 to 120 minutes of oral dosing in the fasted state
Dosage & Administration
- Erectile dysfunction: Recommended dose is 50 mg taken approximately 1 hour before sexual activity, may be increased to a maximum recommended dose of 100 mg or decreased to 25 mg
- Pulmonary arterial hypertension: Recommended dose is 20 mg three times a day
Interaction
Metabolism primarily mediated by cytochrome P450 (CYP) isoforms 3A4 and 2C9, potential for interactions with inhibitors and inducers of these isoenzymes
Contraindications
- Hypersensitivity to any component of this medication
- Concomitant use of organic nitrates

Pregnancy & Lactation
