Indications
- Treatment of erectile dysfunction in men
- Indicated in pulmonary arterial hypertension
Description
- Selective, reversible inhibitor of PDE5
- Increases levels of cGMP in the corpus cavernosum
- Results in smooth muscle relaxation and inflow of blood into penile tissues
Pharmacology
- Involves release of nitric oxide in the corpus cavernosum
- Activates the enzyme guanylate cyclase
- Results in increased levels of cGMP
- Producing smooth muscle relaxation and allowing inflow of blood
Mechanism of Action
- Inhibition of PDE5 by Vigorex
- Enhances the effect of nitric oxide
- No direct relaxant effect on isolated human corpus cavernosum
Pharmacokinetics and Metabolism
- Rapidly absorbed after oral administration
- Mean absolute bioavailability of 41%
- Eliminated predominantly by hepatic metabolism
- Converted to an active metabolite
Dosage & Administration
- Recommended dose of 50 mg for most patients
- May be taken 1 hour before sexual activity
- Maximum recommended dosing frequency is once per day
- Associated factors with increased plasma levels
Interaction
- Metabolism mediated by CYP isoforms 3A4 and 2C9
- Inhibitors of these isoenzymes reduce clearance
- Inducers of these isoenzymes may increase clearance
- Effects of coadministration with CYP3A4 inhibitors
Contraindications
- Hypersensitivity to any component of the medication
- Contraindicated in patients using organic nitrates
Side Effects
- Variety of potential body system reactions and adverse events
Pregnancy & Lactation
- Pregnancy category B
- No adequate and well-controlled studies in pregnant women
- Sildenafil is not indicated for use by women
- No evidence of teratogenicity or embryotoxicity in animal study
Precautions & Warnings
- Evaluation of erectile dysfunction should include determination of underlying causes
- Caution advised when PDE5 inhibitors are co-administered with alpha-blockers
- Consideration for patients with various underlying conditions
Overdose Effects
- Adverse events similar to lower doses but with increased incidence rates and severities
- Standard supportive measures should be adopted as required
- Renal dialysis not expected to accelerate clearance
