Description
- Selective inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5)
- Produces smooth muscle relaxation
- Enhances inflow of blood into penile tissues
Mode of Action
- Involves release of nitric oxide (NO) in the corpus cavernosum during sexual stimulation
- Activates enzyme guanylate cyclase
- Increases levels of cyclic guanosine monophosphate (cGMP)
- Produces smooth muscle relaxation in the corpus cavernosum
- Enhances effect of nitric oxide (NO) by inhibiting phosphodiesterase type 5 (PDE5)
Pharmacokinetics and Metabolism
- Rapidly absorbed after oral administration
- Mean absolute bioavailability of 41%
- Eliminated predominantly by hepatic metabolism
- Converted to an active metabolite with properties similar to the parent
- Terminal half-lives of about 4 hours
Dosage & Administration
- Recommended starting dose of 50 mg, to be taken approximately 1 hour before sexual activity
- Dose may be increased to a maximum recommended dose of 100 mg or decreased to 25 mg
- Maximum recommended dosing frequency is once per day
Interaction
- Metabolism mediated by cytochrome P450 (CYP) isoforms 3A4 and 2C9
- Inhibitors of these isoenzymes may reduce clearance
- Inducers of these isoenzymes may increase clearance
Contraindications
- Hypersensitivity to any component of the medication
- Use with organic nitrates, either regularly or intermittently

- Serious cardiovascular, cerebrovascular, and vascular events reported post-marketing
Pregnancy & Lactation
- Pregnancy category B
- Not indicated for use by women
- No evidence of teratogenicity or embryotoxicity in animal studies
Precautions & Warnings
- Evaluation of erectile dysfunction should include determination of potential underlying causes
- Caution advised when co-administered with alpha-blockers
- Potential for cardiac risk of sexual activity in patients with preexisting cardiovascular disease
- Sensitivity of certain patient groups to the actions of vasodilators including Acmegra
Overdose Effects
- Adverse events similar to those at lower doses but with increased incidence rates and severities
- Standard supportive measures should be adopted
- Renal dialysis not expected to accelerate clearance
