Description
- Selective inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5)
Mode of Action
- Causes smooth muscle relaxation and inflow of blood into the penile tissues
- Involves release of nitric oxide during sexual stimulation
- Enhances the effect of nitric oxide by inhibiting PDE5
- Rapidly absorbed after oral administration
- Cleared by hepatic microsomal isoenzymes
Dosage & Administration
- Recommended dose is 50 mg taken approximately 1 hour before sexual activity
- Dosage may be increased to a maximum recommended dose of 100 mg or decreased to 25 mg based on toleration
- Maximum recommended dosing frequency is once per day
- Recommended dose for pulmonary arterial hypertension is 20 mg three times a day
Interaction
- Metabolism is primarily mediated by cytochrome P450 isoforms 3A4 and 2C9
- Inhibitors or inducers of these isoenzymes may affect Acmegra clearance
- Strong CYP3A4 inhibitors may have significant effects on Acmegra levels
Contraindications
- Hypersensitivity to any component of the medication
- Use of organic nitrates

- Serious cardiovascular, cerebrovascular, and vascular events have been reported post-marketing
