Indications

Description
- Selective inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5)
- Causes smooth muscle relaxation and inflow of blood into penile tissues
- No effect in absence of sexual stimulation
Pharmacology
- Mechanism of action involving release of nitric oxide (NO) during sexual stimulation
- Inhibition of PDE5 by Sildenafil causes increased levels of cGMP
- Rapid absorption after oral administration
- Metabolism by hepatic microsomal isoenzymes
- Excretion in feces and urine
- Reduced clearance in healthy elderly volunteers
Dosage & Administration
- Recommended dose for erectile dysfunction is 50 mg taken 1 hour before sexual activity
- Dose may be increased to a maximum recommended dose of 100 mg or decreased to 25 mg
- Maximum recommended dosing frequency is once per day
- Recommended dose for pulmonary arterial hypertension is 20 mg three times a day
- Factors associated with increased plasma levels of Sildenafil
Interaction
- Metabolism is mediated by CYP3A4 and CYP2C9 isoforms
- Inhibitors of these isoenzymes may reduce Sildenafil clearance
- Inducers of these isoenzymes may increase Sildenafil clearance
- Examples of specific CYP3A4 inhibitors and their effects on Sildenafil
Contraindications
- Hypersensitivity to any component of the medication
- Contraindicated in patients using organic nitrates
Side Effects
- Various adverse effects categorized by body system
- Serious cardiovascular, cerebrovascular, and vascular events reported post-marketing
- Other reported side effects related to nervous system, urogenital system, special senses, etc.
Pregnancy & Lactation
- Pregnancy category B
- No adequate and well-controlled studies in pregnant women
- Sildenafil not indicated for use by women
- Animal study shows no evidence of teratogenicity or embryotoxicity
Precautions & Warnings
- Evaluation of erectile dysfunction should include determination of potential underlying causes
- Potential risk of symptomatic hypotension when co-administered with alpha-blockers
- Considerations for patients stable on alpha-blocker therapy
- Advisory for patients with underlying cardiovascular disease
- Caution in patients with certain conditions, such as anatomical deformation of the penis or bleeding disorders
Overdose Effects
- Incidence rates and severities of adverse events increased with higher doses
- Standard supportive measures should be adopted in cases of overdose
- Renal dialysis not expected to accelerate clearance
