Indications

Description
- Selective inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5)
Mode of Action
- Increases levels of cGMP in corpus cavernosum
- Produces smooth muscle relaxation
- Results in inflow of blood into penile tissues
Pharmacology
- Mechanism of erection involving release of nitric oxide
- Inhibition of PDE5 by Silagra
- Rapid absorption after oral administration
- Mean absolute bioavailability of 41%
- Metabolized by hepatic metabolism
- Converted to an active metabolite
- Maximum observed plasma concentrations reached within 30 to 120 minutes of oral dosing
- Excretion as metabolites predominantly in feces
- No effect in the absence of sexual stimulation
Dosage & Administration
- Recommended starting dose of 50 mg
- Can be taken 4 hours to 0.5 hour before sexual activity
- Maximum recommended dose of 100 mg
- Recommended dose for pulmonary arterial hypertension is 20 mg three times a day
- Increased plasma levels associated with age >65, hepatic impairment, severe renal impairment, and concomitant use of potent cytochrome P450 3A4 inhibitors
Interaction
- Metabolism mediated by cytochrome P450 (CYP) isoforms 3A4 and 2C9
- Inhibitors of CYP3A4 and CYP2C9 may reduce clearance
- Inducers of CYP3A4 and CYP2C9 may increase clearance
- Silagra had no effect on saquinavir pharmacokinetics
- Strong CYP3A4 inhibitors expected to have greater effects
Contraindications
- Hypersensitivity to any component of the medication
- Use of organic nitrates, either regularly or intermittently
Side Effects
- Various body system related side effects including cardiovascular, digestive, hemic and lymphatic, metabolic and nutritional, musculoskeletal, nervous, respiratory, skin and appendages, special senses, and urogenital
- Serious cardiovascular, cerebrovascular, and vascular events reported post-marketing
- Other side effects such as seizure, anxiety, transient global amnesia, prolonged erection, and diplopia
Pregnancy & Lactation
- Category B in pregnancy
- Not indicated for use by women
Precautions & Warnings
- Caution advised when co-administered with alpha-blockers
- Considerations for patients with various underlying conditions
- Potential for cardiac risk of sexual activity in patients with preexisting cardiovascular disease
Overdose Effects
- Adverse events similar to those at lower doses
- Standard supportive measures should be adopted
- Renal dialysis not expected to accelerate clearance
