Indications

Description
- Selective, reversible inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5)
- Requires sexual stimulation for effectiveness
Pharmacology
- Mechanism of Action: Increases cGMP levels in corpus cavernosum
- Pharmacokinetics and Metabolism: Rapid absorption after oral administration, hepatic metabolism, active metabolite similar to parent compound
- Absorption and Distribution: Rapidly absorbed, reduced absorption with high fat meal
- Metabolism and Excretion: Cleared by hepatic microsomal isoenzymes, excreted predominantly in feces
- Pharmacokinetics in Special Populations: Reduced clearance in elderly volunteers
Dosage & Administration
- Recommended dose for erectile dysfunction: 50 mg, taken approximately 1 hour before sexual activity
- Starting dose for patients with certain factors: 25 mg
- Recommended dose for pulmonary arterial hypertension: 20 mg three times a day
Interaction
- Metabolism Mediation: Primarily mediated by cytochrome P450 (CYP) isoforms 3A4 and 2C9
- Inhibitors: Reduced clearance by CYP inhibitors, potentiation of effects by specific inhibitors
Contraindications
- Hypersensitivity to any component of the medication
- Use with organic nitrates
