Description
- Selective, reversible inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5)
- Produces increased levels of cGMP in the corpus cavernosum upon sexual stimulation
- Results in smooth muscle relaxation and increased blood flow into the penile tissues
- Effective treatment for men with erectile dysfunction
- Indicated in pulmonary arterial hypertension
Dosage & Administration
- Recommended starting dose: 50 mg
- May be taken 1 hour before sexual activity
- May be increased to a maximum recommended dose of 100 mg or decreased to 25 mg based on effectiveness and toleration
- Maximum recommended dosing frequency is once per day
Interaction
- Metabolism is mediated by the cytochrome P450 (CYP) isoforms 3A4 and 2C9
- Inhibitors of these isoenzymes may reduce clearance, while inducers may increase clearance
- Potential interactions with CYP3A4 inhibitors, such as ketoconazole, itraconazole, erythromycin, saquinavir
- Potential effects on plasma levels when co-administered with HIV protease inhibitors, alpha-blockers, and endothelin receptor antagonist bosentan
Contraindications
- Hypersensitivity to any component of the medication
- Use of organic nitrates, either regularly or intermittently
Side Effects
- Body as a whole: face edema, photosensitivity reaction, shock, asthenia, pain
- Cardiovascular: angina pectoris, AV block, migraine, syncope
- Digestive: vomiting, glossitis, colitis, dysphagia
- Hemic and Lymphatic: anemia, leukopenia
- Metabolic and Nutritional: thirst, edema, gout, unstable diabetes
- Musculoskeletal: arthritis, arthrosis, myalgia
- Nervous: ataxia, hypertonia, neuralgia, neuropathy
- Respiratory: asthma, dyspnea, laryngitis
- Skin and Appendages: urticaria, herpes simplex, pruritus
- Special Senses: sudden decrease or loss of hearing, mydriasis, conjunctivitis
- Urogenital: cystitis, nocturia, urinary frequency, breast enlargement, urinary incontinence
Pregnancy & Lactation
- Pregnancy category B
- Not indicated for use by women
- Animal studies show no evidence of teratogenicity or embryotoxicity
Precautions & Warnings
- Caution advised when co-administered with alpha-blockers
- Considerations for patients with hemodynamic instability, bleeding disorders, active peptic ulceration, anatomical deformation of the penis
- Potential cardiac risk of sexual activity in patients with preexisting cardiovascular disease
- Sensitivity to the actions of vasodilators including Viax in specific patient groups
- Risk of prolonged erection greater than 4 hours and priapism
- Unknown safety and efficacy in certain patient groups
Overdose Effects
- Adverse events similar to lower doses, but with increased incidence rates and severities
- Standard supportive measures should be adopted in cases of overdose
- Renal dialysis is not expected to accelerate clearance
