Indications
- Treatment of erectile dysfunction in men with sexual stimulation required
- Indicated in pulmonary arterial hypertension
Description
- Selective, reversible inhibitor of cGMP-specific phosphodiesterase type 5 (PDE5)
- Results in smooth muscle relaxation and inflow of blood into penile tissues
Pharmacology
- Mechanism of action involving release of nitric oxide and increased levels of cGMP
- Rapid absorption and metabolism by hepatic P450 enzymes
- Rapidly absorbed with maximum observed plasma concentrations reached within 30 to 120 minutes
Dosage & Administration
- Recommended dose of 50 mg taken approximately 1 hour before sexual activity
- Dose may be increased to a maximum of 100 mg or decreased to 25 mg based on effectiveness and toleration
- Recommended dose for pulmonary arterial hypertension is 20 mg three times a day
Interaction
- Metabolism principally mediated by cytochrome P450 isoforms 3A4 and 2C9
- Potential reduction of clearance by inhibitors of these isoenzymes
- Potential increase in clearance by inducers of these isoenzymes
Contraindications
- Contraindicated in patients with hypersensitivity to any component of the medication
- Contraindicated in patients using organic nitrates
Side Effects
- Wide range of possible side effects including cardiovascular, digestive, metabolic, musculoskeletal, nervous, respiratory, skin, special senses, and urogenital effects
Pregnancy & Lactation
- Pregnancy category B
- Not indicated for use by women
- No evidence of teratogenicity or embryotoxicity in animal studies
Precautions & Warnings
- Caution advised when co-administered with alpha-blockers
- Considerations for patients with preexisting cardiovascular disease
- Potential for cardiac risk of sexual activity
- Advised with caution in patients with anatomical deformation of the penis or conditions predisposing to priapism
- Recommendations for patients with specific underlying conditions
Overdose Effects
- Adverse events similar to lower doses but with increased incidence rates and severities
