Indications
- Erectile Dysfunction (ED)

Pharmacology
Tadalafil is a selective phosphodiesterase type 5 (PDE5) inhibitor. Inhibition of PDE5 increases cGMP in smooth muscle cells, causing smooth muscle relaxation and increased blood flow into the corpus cavernosum, leading to penile erection. It also relaxes smooth muscles in the prostate and bladder, improving blood flow to the urinary tract and widening the opening of the bladder neck.
Dosage & Administration
- For Erectile Dysfunction: Recommended starting dose is 10mg, with the possibility of increasing to 20mg or decreasing to 5mg based on requirement. Maximum dosing frequency is once daily, and effectiveness lasts up to 36 hours.
- For Benign prostatic hyperplasia: The recommended dose is 5mg taken at the same time every day.
- For combined Erectile Dysfunction and Benign prostatic hyperplasia: The recommended dose is 5mg at the same time every day.
Interaction
May interact with Nitrates, Alpha adrenergic blockers, Antihypertensives, Alcohol, Antacids, Ketoconazole, Ritonavir, Erythromycin, Itraconazole, Grapefruit juice, HIV protease inhibitors, Rifampin, Carbamazepine, Phenytoin, and Phenobarbital.
Contraindications
- Use of Nitrates (for example, Nitroglycerine, Isosorbide): may increase hypotensive effects of Nitrates
- Hypersensitivity reactions to Tadalafil
Side Effects
- Common: Headache, Dyspepsia, Back pain, Myalgia, Nasal pharyngitis, Nasal congestion
- Rare: Change in Color Vision, Sudden vision loss, Hearing loss, Stevens-Johnson Syndrome, Exfoliative dermatitis, Angina, Stroke, Myocardial infarction, Severe hypotension, Tachycardia
Pregnancy & Lactation
Tadalafil has been assigned to pregnancy category B by the USFDA. It is only recommended for use during pregnancy when benefit outweighs risk. Caution should be used when administering tadalafil to nursing women.
Precautions & Warnings
Precaution should be taken in conditions such as Angina, renal impairment, hepatic impairment, bleeding concomitant with Nitrates, Alpha Blockers, Alcohol, CYP3A4 Inhibitors, and other PDE5 inhibitors.
