Indications

  • Treatment of mild to moderately severe bacterial infections
  • Infections of the ear, nose, and throat
  • Infections of the lower respiratory tract
  • Infections due to beta-hemolytic streptococci
  • Skin infections
  • Lymphadenitis and lymphangitis
  • Infections of the buccal cavity
  • Prophylaxis of scarlet fever
  • Prevention of recurrences of rheumatic fever
  • Prophylaxis of infection after dental and oral surgical procedures

Pharmacology

  • Absorption from the upper part of the small intestine
  • Acid-stable
  • Best absorbed as potassium salt
  • Convenient means of treating Gram-positive infections
  • Advantage over penicillin G in resistance to gastric acid inactivation

Dosage

  • Determined according to sensitivity of micro-organism and severity of infection
  • 250-500 mg oral dose for adults
  • 125-250 mg oral dose for children above 1 year
  • 62.5-125 mg oral dose for infants below 1 year

Administration

  • Best taken on an empty stomach
  • Tablets swallowed without chewing
  • Syrup shaken vigorously before use

Interaction

  • Reduced absorption with concurrent food intake
  • Potential interactions with methotrexate, probenecid, indomethacin, oral contraceptives, aminoglycosides, oral anticoagulants, warfarin
  • Interference with urine glucose and urobilinogen tests

Contraindications

  • Hypersensitivity to penicillins or any excipients
  • Severe gastrointestinal disorders accompanied by vomiting and diarrhea

Pregnancy & Lactation

  • Crosses the placenta
  • Safe for use during pregnancy
  • Passes into breast milk in small amounts
  • May cause diarrhoea and yeast colonization in infants

Precautions & Warnings

  • Consider cross-allergy between cephalosporins and penicillins
  • Consider potassium content for patients with heart diseases
  • Check for resistant microorganisms
  • Consider sugar content for patients with diabetes mellitus

Overdose Effects

  • Low toxicity
  • Risk of encephalopathy with overdose or renal impairment
  • No acute toxicity with multiple therapeutic dose
  • Elimination through haemodialysis
