Pharmacology

  • Phenoxymethyl penicillin is acid-stable and absorbed from the small intestine.

Dosage

  • Dosage should be determined based on the sensitivity of the micro-organism and severity of infection.
  • Adults: 250-500 mg 6 hourly
  • Children (above 1 year): 125-250 mg 6 hourly
  • Infants (below 1 year): 62.5-125 mg 6 hourly

Administration

  • Best taken with an empty stomach, tablets swallowed without chewing with liquid.

Food Interaction

  • Best taken on an empty stomach to achieve highest absorption rate.

Drug Interactions

  • Concurrent administration with other penicillins may increase methotrexate levels.
  • Concurrent use of certain antibiotics may attenuate penicillin activity.
  • Concurrent use of certain drugs may elevate and prolong serum levels of phenoxymethylpenicillin.

Contraindications

  • Not for patients with hypersensitivity to penicillins or excipients.
  • Not for patients with severe gastrointestinal disorders accompanied by vomiting and diarrhea.

Side Effects

  • May cause hypersensitivity reactions, skin rashes, gastrointestinal disturbances, and hematological changes.

Pregnancy & Lactation

  • Can be used throughout pregnancy if indicated.
  • Passes into breast milk in small amounts, may cause diarrhea and yeast colonization in infant.

Precautions & Warnings

  • Consider cross-allergy between cephalosporins and penicillins.
  • Check potassium content for patients with heart diseases or serious electrolyte disturbances.
  • Regularly check patient's condition for resistant micro-organism proliferation.
  • Consider sugar content for patients with diabetes mellitus.

Overdose Effects

  • Low toxicity, with a broad therapeutic range. Risk of encephalopathy in beta-lactam antibiotic overdose.
