Indications
- Treatment of mild to moderately severe bacterial infections
- Infections of the ear, nose, and throat regions
- Infections of the lower respiratory tract
- Infections due to beta-hemolytic streptococci of group A
- Skin infections
- Lymphadenitis and lymphangitis of bacterial origin
- Prophylaxis of scarlet fever
- Prevention of recurrences of rheumatic fever
- Prophylaxis of infection after dental and oral surgical procedures
- Combination with another appropriate antibiotic may be indicated
Pharmacology
- Phenoxymethyl penicillin is acid-stable
- Absorbed from the upper part of the small intestine
- Best absorbed in the potassium salt form
- Convenient means of treating Gram-positive infections
- Advantage over penicillin G in resistance to inactivation by gastric acid
Dosage
- Dosage determined according to sensitivity of causative micro-organism and severity of infection
- Adult dosage: 250-500 mg 6 hourly
- Children (above 1 year) dosage: 125-250 mg 6 hourly
- Infants (below 1 year) dosage: 62.5-125 mg 6 hourly
- Best taken with an empty stomach
Administration
- Best taken on an empty stomach, preferably one hour before meals
- Tablets are swallowed without chewing with sufficient amounts of liquid
- Syrup bottle has to be shaken vigorously before use
Interaction
- Concurrent intake of food reduces absorption rate
- Concomitant administration of penicillin may increase levels of methotrexate in serum
- Potential drug interactions with bacteriostatic chemotherapeutics or antibiotics
- Concurrent administration of probenecid inhibits renal excretion of penicillins
- Possible interference with laboratory and diagnostic tests
Contraindications
- Contraindicated in patients with hypersensitivity to penicillins or any of the excipients
- Must not be used to treat patients with severe gastrointestinal disorders accompanied by vomiting and diarrhea
Side Effects
- Hypersensitivity reactions involving the skin
- More serious allergic reactions
- Anaphylactic or anaphylactoid reactions
- Skin rashes or inflammation of mucous membranes
- Gastrointestinal disturbances
- Symptoms of enterocolitis
- Changes in blood picture
- Herxheimer's reaction
- Drug-induced aseptic meningitis
- Transient discolouration of the teeth
- Proliferation of resistant microorganisms
Pregnancy & Lactation
- Crosses the placenta
- May be used at any time throughout pregnancy given appropriate indication
- Passes into breast milk in small amounts
- May be used during lactation with precautions for potential infant side effects
Precautions & Warnings
- Consideration of cross-allergy between cephalosporins and penicillins
- Monitoring potassium content for patients with heart diseases or serious electrolyte disturbances
- Regular checking of patient's condition due to potential resistance proliferation
- Consideration of sugar content for patients with diabetes mellitus
- No indication of impaired ability to drive or operate machinery but caution for adverse reactions such as encephalopathy
Overdose Effects
- Low toxicity with broad therapeutic range
- No acute toxicity with multiple therapeutic doses orally
- Risk of encephalopathy in case of overdose or renal impairment
- Special measures in the event of overdosage not required
- Elimination through haemodialysis
