Indications
- Treatment of mild to moderately severe bacterial infections
- Infections of the ear, nose and throat regions
- Lower respiratory tract infections
- Infections due to beta-hemolytic streptococci of group A
- Skin infections
- Lymphadenitis and lymphangitis of bacterial origin
- Infections of the buccal cavity, gums or jaws
- Prophylaxis of scarlet fever
- Prophylaxis of infection after dental and oral surgical procedures
- Prevent recurrences of rheumatic fever
Pharmacology
- Phenoxymethyl penicillin is acid-stable
- Best absorbed from the upper part of the small intestine
- Convenient means of treating Grampositive infections
- Advantage over penicillin G in resistance to inactivation by gastric acid
Dosage
- Dosage determined according to sensitivity of the causative micro-organism and the severity of the infection
- Adult dosage: 250-500 mg 6 hourly
- Children dosage: 125-250 mg 6 hourly
- Infants dosage: 62.5-125 mg 6 hourly
Administration
- Best taken on an empty stomach, preferably one hour before meals
- Tablets are swallowed without chewing with sufficient amounts of liquid
- Syrup requires shaking vigorously before each use
Interaction
- Concurrent intake of food reduces the rate of absorption
- Concomitant administration of penicillins may affect methotrexate levels
- Disturbed absorption of other orally administered drugs if diarrhoea occurs
- Concurrent use with bacteriostatic chemotherapeutics or antibiotics may attenuate or abolish penicillin activity
- Interference with oral contraceptives is uncertain
- Intestinal sterilization with aminoglycosides may reduce absorption
- Combined use with oral anticoagulants may prolong prothrombin time/INR
- Interference with certain laboratory and diagnostic tests
Contraindications
- Must not be administered in patients with hypersensitivity to penicillins or any of the excipients
- Must not be used to treat patients with severe gastrointestinal disorders accompanied by vomiting and diarrhea
Side Effects
- Hypersensitivity reactions involving the skin
- Allergic reactions such as drug fever, vasculitis, serum sickness or interstitial nephritis
- Anaphylactic or anaphylactoid reactions
- Skin rashes or inflammation of mucous membranes
- Gastrointestinal disturbances
- Diarrhoea, enterocolitis, pseudomembranous colitis
- Changes in blood picture
- Herxheimer's reaction, drug-induced aseptic meningitis
- Transient discolouration of the teeth
Pregnancy & Lactation
- Crosses the placenta
- May be used at any time throughout pregnancy
- Passes into breast milk in small amounts
- May be used during lactation with potential effects on the infant
Precautions & Warnings
- Consider cross-allergy between cephalosporins and penicillins
- Consider potassium content when treating patients with heart diseases or electrolyte disturbances
- Predisposes the patient to encephalopathy risk
- May lead to the proliferation of resistant micro-organisms
- No indication of impaired ability to drive or operate machinery
Overdose Effects
- Low toxicity with broad therapeutic range
- Risk of encephalopathy in cases of administration of beta-lactam antibiotics
- Elimination can be accomplished through haemodialysis
