Indications
- Treatment of bacterial infections in ear, nose, and throat regions
- Treatment of lower respiratory tract infections
- Treatment of infections due to beta-hemolytic streptococci
- Treatment of skin infections
- Prophylaxis of scarlet fever
- Prevention of recurrences of rheumatic fever
- Prophylaxis of infection after dental and oral surgical procedures
- Combination with other appropriate antibiotics may be indicated
Pharmacology
- Absorption from upper part of small intestine
- Best absorbed form is potassium salt
- Convenient for treating Gram-positive infections
- Advantage over penicillin G in resistance to gastric acid inactivation
Dosage
- Dosage determined by sensitivity of causative micro-organism and infection severity
- Adult dosage: 250-500 mg 6 hourly
- Children dosage: 125-250 mg 6 hourly
- Infants dosage: 62.5-125 mg 6 hourly
Administration
- Best taken on an empty stomach
- Tablets swallowed without chewing, with liquid
- Syrup shaken vigorously before each use
Interaction
- Reduced absorption rate with concurrent food intake
- Potential drug interactions with methotrexate, bacteriostatic antibiotics
- Inhibition of renal excretion with concurrent probenecid use
- Possible interference with oral contraceptives effectiveness
Contraindications
- Hypersensitivity to penicillins or excipients
- Severe gastrointestinal disorders with vomiting and diarrhea
Side Effects
- Hypersensitivity reactions involving the skin
- Serious allergic reactions like drug fever, vasculitis
- Anaphylactic or anaphylactoid reactions
- Skin rashes, mucous membrane inflammation, dryness of the mouth
- Gastrointestinal disturbances, diarrhoea, enterocolitis
- Reduction in blood cell counts, myelosuppression
- Drug-induced aseptic meningitis, encephalopathy risk
Pregnancy & Lactation
- Crosses the placenta, can be used during pregnancy
- Passes into breast milk in small amounts, may be used during lactation
Precautions & Warnings
- Consider cross-allergy with cephalosporins
- Consider potassium content in patients with heart diseases or electrolyte disturbances
- Check patient's condition at regular intervals
- Consider sugar content in patients with diabetes
