Indications

  • Treatment of bacterial infections
  • Infections of the ear, nose, and throat
  • Infections of the lower respiratory tract
  • Skin infections
  • Prophylaxis of scarlet fever
  • Prophylaxis of infection after dental procedures

Pharmacology

  • Acid-stable
  • Absorbed from upper part of small intestine
  • Best absorbed in potassium salt form
  • Convenient for treating Gram-positive infections
  • Resistant to inactivation by gastric acid

Dosage

  • Dependent on sensitivity of micro-organism
  • Adjusted to clinical response
  • Adult dosage: 250-500 mg 6 hourly
  • Children dosage: 125-250 mg 6 hourly
  • Infants dosage: 62.5-125 mg 6 hourly

Administration

  • Best taken on empty stomach
  • Tablets should be swallowed without chewing
  • Syrup requires vigorous shaking before use

Interaction

  • Concurrent intake of food can reduce absorption
  • Increased levels of methotrexate in serum
  • Disturbed absorption of other orally administered drugs in case of diarrhea
  • Attenuation of penicillin activity in concurrent use with bacteriostatic antibiotics
  • Inhibition of renal excretion of penicillins in concurrent use with probenecid

Contraindications

  • Hypersensitivity to penicillins or excipients
  • Severe gastrointestinal disorders with vomiting and diarrhea

  • Anaphylactic or anaphylactoid reactions
  • Skin rashes or inflammation of mucous membranes

  • Transient discolouration of teeth
  • Proliferation of resistant microorganisms

Pregnancy & Lactation

  • Crosses the placenta
  • Passes into breast milk in small amounts
  • May be used throughout pregnancy and lactation, with consideration of potential effects on the infant

Precautions & Warnings

  • Possibility of cross-allergy between cephalosporins and penicillins
  • Consideration of potassium content in heart patients or electrolyte disturbances
  • Encephalopathy risk in case of overdose or renal impairment
  • Regular checking of patient's condition
  • Monitoring for secondary infection
  • Consideration of sugar content in diabetic patients

Overdose Effects

  • Low toxicity with broad therapeutic range
  • Risk of encephalopathy in case of overdose or renal impairment
  • Special measures not required for overdosage
  • Elimination through haemodialysis
