Indications
- Treatment of bacterial infections of ear, nose, and throat
- Treatment of infections of lower respiratory tract
- Treatment of infections due to beta-hemolytic streptococci
- Treatment of skin infections
- Treatment of lymphadenitis and lymphangitis
- Prophylaxis of scarlet fever
- Prophylaxis of rheumatic fever
- Prophylaxis of infection after dental and oral surgical procedures

Pharmacology
- Acid-stable and absorbed from the upper part of the small intestine
- Best absorbed in potassium salt form
- Convenient for treating Gram-positive infections
- Resistant to inactivation by gastric acid
Dosage
- Dosage determined by sensitivity of micro-organism and severity of infection
- Adult dosage: 250-500 mg 6 hourly
- Children dosage: 125-250 mg 6 hourly
- Infants dosage: 62.5-125 mg 6 hourly
- Best taken with an empty stomach
Administration
- Best taken on an empty stomach
- Tablets swallowed without chewing with liquid
- Syrup to be shaken vigorously before use
Interaction
- Reduced absorption with concurrent food intake
- Increased levels of methotrexate in serum with concomitant penicillin administration
- Disturbed absorption of other orally administered drugs in case of diarrhoea
- Attenuation of penicillin activity with bacteriostatic chemotherapeutics or antibiotics
- Inhibited renal excretion of penicillins with concurrent probenecid use
- Prolonged serum levels of phenoxymethylpenicillin with certain drugs
- Transient reduction in plasma concentrations of oestrogens and gestagens
- Reduced absorption with intestinal sterilization with aminoglycosides
- Prolonged prothrombin time/INR with combined use of penicillins and oral anticoagulants
- Interference with non-enzymatic urine glucose determinations and tests for urobilinogen
Contraindications
- Hypersensitivity to penicillins
- Severe gastrointestinal disorders accompanied by vomiting and diarrhea
Side Effects
- Hypersensitivity reactions involving the skin
- Allergic reactions including anaphylactic reactions
- Skin rashes or inflammation of mucous membranes
- Gastrointestinal disturbances
- Diarrhoea, including pseudomembranous colitis
- Changes in blood picture
- Herxheimer's reaction during spirochaetal infections
- Drug-induced aseptic meningitis
- Transient discolouration of the teeth
- Proliferation of resistant microorganisms
- Risk of encephalopathy, particularly in case of overdose or renal impairment
Pregnancy & Lactation
- Crosses the placenta
- May be used throughout pregnancy with appropriate indication
- Passes into breast milk in small amounts
- May be used during lactation, but may cause diarrhoea and yeast colonization in the infant
Precautions & Warnings
- Possibility of cross-allergy between cephalosporins and penicillins
- Consideration of potassium content in patients with heart diseases or serious electrolyte disturbances
- Proliferation of resistant microorganisms with prolonged antibiotic use
- Regular checking of patient's condition
- Consideration of sugar content in patients with diabetes mellitus
- No indication of impaired ability to drive or operate machinery
Overdose Effects
- Low toxicity with a broad therapeutic range
- Risk of encephalopathy in case of beta-lactam antibiotic administration, particularly in case of overdose or renal impairment
- Elimination can be accomplished through haemodialysis
