Indications
- Treatment of mild to moderately severe bacterial infections
- Infections of the ear, nose, and throat regions
- Infections of the lower respiratory tract
- Infections due to beta-hemolytic streptococci
- Skin infections
- Prophylaxis of scarlet fever
- Prevention of recurrences of rheumatic fever
- Prophylaxis of infection after dental and oral surgical procedures
Pharmacology
- Acid-stable
- Absorbed from the upper part of the small intestine
- Best absorbed as potassium salt
- Convenient for treating Gram-positive infections
- Advantage over penicillin G in resistance to inactivation by gastric acid
Dosage
- Dosage determination based on sensitivity of causative micro-organism and severity of infection
- Adult dosage: 250-500 mg 6 hourly
- Children dosage: 125-250 mg 6 hourly, with variations for different age groups
- Best taken with an empty stomach
Administration
- Best taken on an empty stomach
- Tablets swallowed without chewing with sufficient amounts of liquid
- Syrup to be shaken vigorously before each use
Interaction
- Reduced absorption when taken with food
- Concomitant administration with penicillins may affect methotrexate levels
- Disturbed absorption of other orally administered drugs if diarrhoea occurs
- Attenuation of penicillins' activity when combined with bacteriostatic chemotherapeutics or antibiotics
- Concurrent use with probenecid inhibits renal excretion of penicillins
- Potential impact on oral contraceptives' effectiveness
Contraindications
- Hypersensitivity to penicillins or any excipients
- Severe gastrointestinal disorders accompanied by vomiting and diarrhea

- Hypersensitivity reactions involving the skin
- More serious allergic reactions like drug fever, vasculitis, or interstitial nephritis
- Severe bullous skin reactions
- Gastrointestinal disturbances
- Diarrhoea as a symptom of enterocolitis
- Reduction in blood cell counts

Pregnancy & Lactation
- Crosses the placenta
- Passes into breast milk in small amounts

Precautions & Warnings
- Cross-allergy between cephalosporins and penicillins
- Consideration of potassium content in patients with heart diseases or serious electrolyte disturbances
- Encephalopathy risk in case of overdose or renal impairment
- Possible proliferation of resistant micro-organisms with prolonged antibiotic administration
- Monitoring the patient's condition at regular intervals
- Taking sugar content into consideration in patients with diabetes mellitus

- Risk of encephalopathy in cases of beta-lactam antibiotic administration
- Elimination through haemodialysis if necessary
