Indications

  • Treatment of mild to moderately severe bacterial infections
  • Infections of the ear, nose, and throat regions
  • Infections of the lower respiratory tract
  • Infections due to beta-hemolytic streptococci
  • Skin infections
  • Lymphadenitis and lymphangitis
  • Prophylaxis of scarlet fever
  • Prevention of recurrences of rheumatic fever
  • Prophylaxis of infection after dental and oral surgical procedures
  • Combination therapy with other appropriate antibiotics

Pharmacology

  • Acid-stable and absorbed from the upper part of the small intestine
  • Best absorbed as potassium salt
  • Convenient for treating Gram-positive infections
  • Advantage over penicillin G in resistance to gastric acid inactivation

Dosage

  • Dosage determination based on sensitivity of causative micro-organism and severity of infection
  • Adult dosage: 250-500 mg 6 hourly
  • Children dosage: 125-250 mg 6 hourly
  • Infants dosage: 62.5-125 mg 6 hourly
  • Best taken with an empty stomach

Administration

  • Best taken on an empty stomach
  • Tablets swallowed without chewing with sufficient liquid
  • Syrup to be shaken vigorously before use

Interaction

  • Concurrent intake of food reduces rate of absorption
  • Potential drug interactions with methotrexate, bacteriostatic chemotherapeutics, probenecid, indomethacin, oral contraceptives, aminoglycosides, oral anticoagulants
  • Interference with laboratory and diagnostic tests

Contraindications

  • Hypersensitivity to penicillins or any excipients
  • Severe gastrointestinal disorders accompanied by vomiting and diarrhea

Side Effects

  • Hypersensitivity reactions involving the skin
  • Allergic reactions such as drug fever, vasculitis, interstitial nephritis
  • Anaphylactic or anaphylactoid reactions
  • Skin rashes or inflammation of mucous membranes
  • Gastrointestinal disturbances
  • Diarrhoea as a symptom of enterocolitis
  • Changes in blood picture
  • Herxheimer's reaction
  • Transient discolouration of teeth
  • Proliferation of resistant microorganisms
  • Encephalopathy risk

Pregnancy & Lactation

  • Crosses the placenta
  • Passes into breast milk in small amounts
  • Use during pregnancy with appropriate indication
  • Use during lactation with potential side effects in infant

Precautions & Warnings

  • Possibility of cross-allergy with cephalosporins
  • Consideration of potassium content in patients with heart diseases
  • Regular intervals checking of patient's condition
  • Conducting appropriate measures in case of secondary infection
  • Consideration of sugar content in patients with diabetes mellitus
  • No indication of impaired ability to drive or operate machinery
  • Encephalopathy risk precautions

Overdose Effects

  • Low toxicity with broad therapeutic range
  • Risk of encephalopathy with beta-lactam antibiotics
  • Elimination via haemodialysis
