Medicine Details

Indications

  • Treatment of mild to moderately severe bacterial infections
  • Infections of the ear, nose and throat regions
  • Infections of the lower respiratory tract
  • Infections due to beta-hemolytic streptococci
  • Skin infections
  • Lymphadenitis and lymphangitis
  • Prophylaxis of scarlet fever
  • Prevention of recurrences of rheumatic fever
  • Prophylaxis of infection after dental and oral surgical procedures
  • Combination with another appropriate antibiotic may be indicated

Pharmacology

  • Absorbed from the upper part of the small intestine
  • Best absorbed as the potassium salt
  • Administration with meals
  • Resistance to inactivation by gastric acid

Dosage

  • Determined according to the sensitivity of the causative micro-organism and the severity of the infection
  • Adjusted to the clinical response of the patient
  • Specific dosages for adults, children, and infants
  • Best taken with an empty stomach

Administration

  • Best taken on an empty stomach
  • Tablets are swallowed without chewing
  • Syrup must be shaken vigorously before use

Interaction

  • Concurrent intake of food reduces the rate of absorption
  • Concomitant administration with other drugs may lead to increased serum levels
  • Monitoring of methotrexate serum levels necessary
  • Diarrhea may disturb absorption of other orally administered drugs
  • Attenuation or abolition of penicillin activity when combined with bacteriostatic chemotherapeutics or antibiotics
  • Inhibition of renal excretion by probenecid
  • Possible elevation and prolonged serum levels with concurrent use of certain drugs
  • Reduction in plasma concentrations of oestrogens and gestagens
  • Reduction in absorption with intestinal sterilization
  • Prolongation of prothrombin time/INR with oral anticoagulants
  • Interference with urine glucose determinations and urobilinogen tests

Contraindications

  • Hypersensitivity to penicillins or any excipients
  • Severe gastrointestinal disorders accompanied by vomiting and diarrhea

  • Allergic reactions
  • Anaphylactic or anaphylactoid reactions
  • Skin rashes or inflammation of mucous membranes
  • Gastrointestinal disturbances
  • Diarrhoea including pseudomembranous colitis
  • Reduction in blood picture
  • Herxheimer's reaction
  • Drug-induced aseptic meningitis
  • Transient discolouration of the teeth
  • Proliferation of resistant micro organisms
  • Increased encephalopathy risk

Pregnancy & Lactation

  • Crosses the placenta
  • Passes into the breast milk in small amounts
  • Usage during pregnancy and lactation

Precautions & Warnings

  • Cross-allergy between cephalosporins and penicillins
  • Consideration of potassium content in patients with heart diseases or serious electrolyte disturbances
  • Monitoring for the proliferation of resistant micro-organisms
  • Consideration of sugar content in patients with diabetes mellitus
  • No impairment of ability to drive or operate machinery
  • Increased encephalopathy risk in case of adverse reactions

Overdose Effects

  • Low toxicity and broad therapeutic range
  • Risk of encephalopathy with overdose or renal impairment
  • Elimination through haemodialysis
