Indications
- Depressive Illness: particularly where sedation is required

Description
- Tricyclic antidepressant
- Marked anticholinergic and sedative properties
- Prevents reuptake of noradrenaline and serotonin
- Rapid absorption from the Gl tract
- Excretion in the urine
Pharmacology
- Inhibition of membrane pump mechanism for re-uptake of transmitter amines
- Increase in concentration of norepinephrine and serotonin at synaptic clefts
- Regulation of mood
- Monoamine hypothesis in depression
- Counteraction of mechanisms leading to depressive effects
Dosage & Administration
- Initial dosage for depression: 75 mg (Elderly and Adolescents 30-75 mg) daily
- Nocturnal Enuresis dosage for children: 10-20 mg for 7-10 years, 25-50 mg for 11-16 years
- Prophylaxis of Migraine dosage: 100 mg daily
- Tension Headache dosage: 10-25 mg three times daily
Interaction
- Enhancement of sedative effect of alcohol and opioid analgesics
- Increased risk of ventricular arrhythmias with Moxifloxacin or Terfenadine
- Inhibition of metabolism by Disulfiram and Cimetidine
- Enhanced risk of postural hypotension with diuretics
Contraindications
- Myocardial infarction
- Arrhythmias, particularly heart block of any degree
- Mania
- Severe liver disease
Side Effects
- Anticholinergic effects: excessive perspiration, dry mouth, blurred vision
- Cardiovascular reactions: hypotension, syncope, postural hypotension, hypertension
- CNS and Neuromuscular effects: confusional states, disturbed concentration, disorientation
- Allergic reactions: skin rash, urticaria, photosensitization
- Haematological effects: bone-marrow depression
- Gastrointestinal effects: nausea, vomiting, weight loss, diarrhea, constipation
- Endocrine effects: testicular swelling, gynaecomastia, galactorrhoea
Pregnancy & Lactation
Pregnancy Category C. Crossing the placenta. Excretion into breast milk. A decision should be made whether to discontinue nursing or the drug.
Precautions & Warnings
- Use with caution in patients with epilepsy, glaucoma, urinary retention, cardiac disease, diabetes, pregnancy, hepatic impairment, thyroid disease, increased intra-ocular pressure, and psychoses
