Description
- Tryptin is a tricyclic antidepressant with marked anticholinergic and sedative properties.
Indications
- Depressive Illness particularly where sedation is required
- Nocturnal Enuresis in children
- Prophylaxis of Migraine
- Tension Headache
- Chronic Pain
Pharmacology
- Inhibits the re-uptake of transmitter amines such as norepinephrine and serotonin
- Increases concentration of neurotransmitters at synaptic clefts in the brain
- Regulates mood by countering deficiencies in serotonin and norepinephrine neurotransmission
Dosage & Administration
- Depression: Initially 75 mg daily, increased gradually to 150-200 mg
- Nocturnal Enuresis: Child 7-10 years: 10-20 mg, 11-16 years: 25-50 mg at night; max. period of treatment 3 months
- Prophylaxis of Migraine: 100 mg daily
- Tension Headache: 10-25 mg three times daily
Interaction
- Enhances sedative effect of alcohol and opioid analgesics
- Increases risk of ventricular arrhythmias when used with Moxifloxacin or Terfenadine
- Inhibited by Disulfiram and Cimetidine
- Enhances risk of postural hypotension when used with diuretics
Contraindications
- Myocardial infarction
- Arrhythmias
- Severe liver disease
- Sedation affecting the ability to drive or operate machinery
Side Effects
- Anticholinergic: Excessive perspiration, dry mouth, blurred vision, hyperpyrexia, urinary retention
- Cardiovascular reactions: Hypotension, syncope, postural hypotension, hypertension, tachycardia, palpitations
- CNS and Neuromuscular: Confusional states, disorientation, delusions
- Allergic: Skin rash, urticaria, photosensitization
- Haematological: Bone-marrow depression
- Gastrointestinal: Nausea, vomiting, diarrhoea, constipation
- Endocrine: Testicular swelling, gynaecomastia, breast enlargement
Pregnancy & Lactation
- Pregnancy Category C
- Crosses the placenta
- Excreted into breast milk
- Use during pregnancy only if potential benefit justifies potential risk to fetus
- Consideration should be made for nursing infants if continuing the drug
Precautions & Warnings
- Use with caution in patients with a history of epilepsy, glaucoma, urinary retention, cardiac disease, diabetes, hepatic impairment, thyroid disease, increased intra-ocular pressure, and psychoses
