Indications

  • Skin and soft tissue infections
  • Respiratory tract infections
  • Osteomyelitis
  • Enteritis
  • Endocarditis
  • Urinary tract infection
  • Meningitis
  • Septicaemia
  • Prophylactic agent for surgery

Pharmacology

  • Active against Gram-positive organisms
  • Little activity against Gram-negative bacilli
  • Inhibits formation of bacterial cell wall
  • Bactericidal antibiotic
  • Useful against penicillinase-producing staphylococci

Dosage

  • Oral administration for Adults: 250 mg four times daily
  • Dosage may be doubled in severe infections
  • Oral administration for Children (2-10 years): Half of adult dose
  • Parenteral administration for Adults: 250 mg-1 g four times daily
  • Intramuscular Injection: 250 mg four times daily
  • Intravenous Injection: 250 mg-1 g four times daily
  • Oral doses should be administered 1 hour before meal

Interaction

  • Concurrent use may result in increased level of the drug in blood

Contraindications

  • Contraindicated in penicillin hypersensitive patients

Side Effects

  • Common gastrointestinal side effects: nausea, vomiting, diarrhoea, dyspepsia
  • Other side effects: rashes, urticaria, purpura, fever, interstitial nephritis, hepatitis, cholestatic jaundice

Pregnancy & Lactation

  • US FDA Pregnancy Category B
  • Use during pregnancy only if clearly needed
  • Excreted in human milk
  • Caution in lactating mothers

Precautions & Warnings

  • Use with caution in patients with hepatic dysfunction
  • Caution in the treatment of patients with allergic diathesis

Use in Special Populations

  • Consider dose reduction or extension in severe renal failure

Reconstitution

  • Administered in other routes in conjunction with systemic therapy
  • Dissolved in a 0.5% solution of lignocaine hydrochloride for intraarticular injection
  • Administered by intrapleural injection
  • Dissolved in sterile water and inhaled by nebulizer
