Indications
- Essential hypertension
- Angina pectoris
- Vasospastic angina
Mode of Action
- Dihydropyridine calcium-channel blocker
- Long duration of action
- Vasodilation
- Reduced peripheral resistance
- Increased coronary blood flow
- Reflex increase in coronary heart rate
Dosage & Administration
- Usual dose of 5 mg once daily for hypertension
- Maximum dose of 10 mg once daily
- Lower starting dose for elderly and hepatic insufficiency
- 5 to 10 mg for angina
- May be taken without regard to meals
Drug Interaction
- Potentially hazardous interactions with p-blockers
- Absence of interaction with Digoxin
- Absence of interaction with Cimetidine
- No significant alteration with Warfarin
- No effect of food on absorption
Contraindications
- Hypersensitivity to dihydropyridine derivatives
- Pregnancy

- Dizziness
- Flushing
- Headache
- Hypotension
- Peripheral edema
- Gastrointestinal disturbances
- Increased micturition frequency
- Lethargy
- Eye pain
- Mental depression
- Ischaemic chest pain
- Excessive fall in blood pressure

- Abnormalities in liver function
Pregnancy & Lactation
- Pregnancy Category C
- Use only if potential benefit justifies potential risk to fetus
- Discontinue nursing while administered
Precautions & Warnings
- Precaution in patients with hepatic impairment
- Precaution during pregnancy and breastfeeding
Use in Special Populations
- Dosage recommendations for children with hypertension
- Effect on blood pressure in children under 6 years not known
- Normal dosage regimens recommended for elderly
- Normal dosage recommended for renal impairment
- Cautious dose selection in hepatic impairment
- Initiate at lowest dose for severe hepatic impairment
Overdose Effects
- Excessive peripheral vasodilatation
- Reflex tachycardia
- Systemic hypotension up to shock
- Clinically significant hypotension calls for active cardiovascular support
- Elevation of extremities
- Attention to circulating fluid volume
- Vasoconstrictor may be helpful
- Intravenous calcium gluconate may be beneficial
- Gastric lavage may be worthwhile
- Charcoal use within 2 hours
- Dialysis not likely to be of benefit
