Indications

  • Treatment of ear, nose, and throat infections
  • Treatment of lower respiratory tract infections
  • Treatment of skin and soft tissue infections
  • Treatment of genito-urinary tract infections
  • Treatment of venereal disease
  • Treatment of dental abscess

Pharmacology

  • Broad spectrum penicillin
  • Effective against Gram-positive and Gram-negative bacteria
  • Inhibits biosynthesis of cell wall
  • Stable in the presence of gastric acid
  • Rapidly absorbed after oral administration
  • Widely distributed in body tissues and fluids

Dosage

  • 250 mg three times daily for adults
  • 500 mg three times daily for severe infections in adults
  • 125 mg three times daily for children (up to 10 years of age)
  • 250 mg three times daily for severe infections in children
  • 3 gm every 12 hours for severe or recurrent purulent respiratory infection
  • 1 g three times daily for otitis media in adults
  • 40 mg/kg body weight daily in 3 divided doses for otitis media in children
  • 500-1000 mg three times daily for pneumonia
  • 3 gm for dental abscess

Administration

  • Reconstituted suspension can be administered by adding it to milk, fruit juice, or water
  • Preparations should be taken immediately after reconstitution

Interaction

  • Concurrent use with Probenecid may increase and prolong blood levels
  • May affect gut flora, leading to lower estrogen reabsorption and reduced efficacy of combined oral contraceptives

Contraindications

  • Contraindicated in penicillin hypersensitive patients

Side Effects

  • Mild and transient side effects
  • May include diarrhea, indigestion, or rash
  • Rare reports of pseudo-membranous colitis

Pregnancy & Lactation

  • US FDA pregnancy category B
  • No adequate and well-controlled studies in pregnant women
  • Excreted in human milk
  • Caution should be exercised when administered to lactating mothers

Precautions & Warnings

  • Possibility of superinfections with mycotic or bacterial pathogens
  • If superinfections occur, medication should be discontinued and appropriate therapy instituted
