Pharmacology

Amoxicillin is a broad spectrum penicillin effective against a wide range of bacteria. It acts through the inhibition of cell wall biosynthesis and is rapidly absorbed after oral administration.

Dosage

  • Adult: 250 mg three times daily, increasing to 500 mg three times daily for severe infections
  • Children: 125 mg three times daily, increasing to 250 mg three times daily for severe infections
  • Other recommendations:
    • Severe or recurrent purulent respiratory infection: 3 gm every 12 hours
    • Otitis media: 1 g three times daily for adults, 40 mg/kg body weight daily in 3 divided doses for children (max. 3 g daily)
    • Pneumonia: 500-1000 mg three times daily
    • Dental abscess: 3 gm, repeated after 10-12 hours
    • Urinary tract infections: 3 gm, repeated after 10-12 hours
    • Gonorrhoea: Single dose of 2-3 gm with Probenecid 1 gm (Probenecid is contraindicated in children under 2 years)
  • Renal impairment: May require reduction in total daily dosage

Administration

Reconstituted suspension can be administered by adding to milk, fruit juice, or water and should be taken immediately.

Interaction

  • Concurrent use with Probenecid may result in increased and prolonged blood levels
  • May affect gut flora and reduce efficacy of combined oral estrogen/progesterone contraceptives

Contraindications

Contraindicated in penicillin hypersensitive patients

  • Diarrhoea
  • Indigestion
  • Rash (occasional)
  • Pseudo-membranous colitis (rare)

Pregnancy lactation

  • Pregnancy category: US FDA pregnancy category B
  • Caution lactating mothers: Caution should be exercised when administered to a lactating mother

Precautions warnings

Keep in mind the possibility of superinfections with mycotic or bacterial pathogens during therapy, and discontinue if superinfections occur

  • Intramuscular: Add 2.5 ml water for injection to Amoxycillin 500 mg injection vial
  • Intravenous: Dissolve in 10 ml water for injection
