Description
- Amlodipine is a dihydropyridine calcium-channel blocker used for the treatment of hypertension and angina pectoris.
Dosage and administration
- Usual dose is 5 mg once daily for hypertension. 5 to 10 mg for angina. May be taken without regard to meals.
Pharmacology
- Amlodipine influences myocardial cells, conducting system cells of the heart, and cells of vascular smooth muscle. It results primarily in vasodilation, reduced peripheral resistance, increased coronary blood flow, and increased myocardial oxygen supply and cardiac output.
Indications
- Treatment of essential hypertension, chronic stable angina pectoris, and confirmed or suspected vasospastic angina
Interaction
- Potential interactions with p-blockers in patients with markedly impaired left ventricular function. No interaction with Digoxin, Cimetidine, Warfarin, or food altering absorption.
Contraindications
- Hypersensitivity to dihydropyridine derivatives, pregnancy
Side effects
- Adverse effects include dizziness, flushing, headache, hypotension, peripheral edema, gastrointestinal disturbances, increased micturition frequency, lethargy, eye pain, mental depression, paradoxical increase in ischaemic chest pain, excessive fall in blood pressure, rashes, fever, and abnormalities in liver function.
Pregnancy and lactation
- Pregnancy Category C; should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether Amlodipine is excreted in human milk.
Precautions and warnings
- Precautions should be taken in patients with hepatic impairment, during pregnancy, and breastfeeding.
Use in special populations
- Specific dosing for children with hypertension aged 6 to 17 years, lack of information for children under 6 years, recommended dosage regimens for the elderly, normal dosage recommended for renal impairment, caution needed in hepatic impairment.
Overdose effects
- Symptoms of large overdosage, management includes active cardiovascular support, potential use of a vasoconstrictor, intravenous calcium gluconate, gastric lavage, and charcoal for reducing absorption rate.
