Indications
- Essential hypertension

Pharmacology
- Dihydropyridine calcium-channel blocker
- Long duration of action
- Influences myocardial cells
- Affects cells within the specialized conducting system of the heart
- Affects cells of vascular smooth muscle
- Results in vasodilation
- Reduces peripheral resistance
- Increases coronary blood flow
- Increases coronary heart rate
Dosage & Administration
- Usual dose is 5 mg once daily for hypertension
- Maximum dose is 10 mg once daily for hypertension
- Lower dose for elderly and patients with hepatic insufficiency
- May be taken without regard to meals
Interaction
- Potentially hazardous interactions with p-blockers
- No interaction with Digoxin
- No interaction with Cimetidine
- CCB did not significantly alter the effect of Warfarin on prothrombin time
- Food does not alter the rate or extent of absorption
Contraindications
- Hypersensitivity to dihydropyridine derivatives
- Pregnant women
Side Effects
- Dizziness
- Flushing
- Headache
- Hypotension
- Peripheral edema
- Gastrointestinal disturbances
- Increased micturition frequency
- Lethargy
- Eye pain
- Mental depression
- Possible paradoxical increase in ischaemic chest pain
- Possible excessive fall in blood pressure leading to cerebral or myocardial ischaemia or transient blindness
- Rashes
- Fever
- Abnormalities in liver function
Pregnancy & Lactation
- Pregnancy Category C
- Recommendation to discontinue nursing while Amlodipine is administered
Precautions & Warnings
- Precautions in patients with hepatic impairment
- Precautions during pregnancy and breastfeeding
Use in Special Populations
- Dosage for children with hypertension from 6 to 17 years of age
- Effect on blood pressure in patients less than 6 years of age is not known
- Normal dosage regimens recommended in the elderly
- No correlation between changes in plasma concentrations and degree of renal impairment
- Normal dosage recommended for renal impairment
- No established dosage recommendations for mild to moderate hepatic impairment
- Lowest dose recommended for severe hepatic impairment
Overdose Effects
- Excessive peripheral vasodilatation
- Reflex tachycardia
- Marked and prolonged systemic hypotension
- Clinical management for hypotension due to amlodipine overdosage
- Potential benefit of vasoconstrictor in restoring vascular tone and blood pressure
- Beneficial effects of intravenous calcium gluconate in reversing the effects of calcium channel blockade
