Indications

  • Essential hypertension
  • Angina pectoris
  • Vasospastic angina

Pharmacology

  • Dihydropyridine calcium-channel blocker
  • Vasodilation
  • Myocardial oxygen supply
  • Coronary blood flow
  • Cardiac output

Dosage & Administration

  • Usual dose is 5 mg once daily
  • Maximum dose is 10 mg once daily
  • May be taken without regard to meals

Interaction

  • Potentially hazardous interactions with p-blockers in patients with impaired cardiac left ventricular function
  • No interaction with Digoxin documented in clinical study
  • No interaction with Cimetidine in healthy volunteers
  • No significant alteration of Warfarin effect on prothrombin time
  • Food does not alter the absorption

Contraindications

  • Hypersensitivity to dihydropyridine derivatives
  • Pregnant women

Pregnancy & Lactation

  • Use only if potential benefit justifies potential risk to fetus
  • Recommendation to discontinue nursing

Precautions & Warnings

  • Precaution in patients with hepatic impairment, pregnancy, and breastfeeding

Use in Special Populations

  • Specific dosing for children with hypertension from 6 to 17 years of age
  • Limited information on the effect on blood pressure in patients less than 6 years old
  • Normal dosage regimens recommended in the elderly
  • Recommended normal dosage in renal impairment
  • Caution in dosage selection for mild to moderate hepatic impairment
  • Initiation at lowest dose and slow titration in severe hepatic impairment

Overdose Effects

  • Excessive peripheral vasodilatation
  • Reflex tachycardia
  • Marked and prolonged systemic hypotension
  • Clinically significant hypotension calls for active cardiovascular support
  • Elevation of extremities
  • Attention to circulating fluid volume and urine output
  • Use of vasoconstrictor and intravenous calcium gluconate for management
  • Possibility of benefit from gastric lavage and charcoal use
