Indications
- Treatment of infections caused by mecillinam-sensitive organisms
- Acute cystitis
- Complicated urinary tract infections
- Salmonellosis
- Shigellosis
- Enteropathic E. coli diarrhoea
- Gram-negative septicaemia
- Biliary infections
Pharmacology
- Belongs to amdinopenicillin class
- Prodrug formulation of mecillinam
- Bactericidal action
- Interferes with bacterial cell wall synthesis
- Active against Enterobacteriaceae
- Poor activity against Gram-positive organisms
- Poor activity against Pseudomonas aeruginosa
- Practically no activity against Enterococcus faecalis
Dosage
- Usual adult dose of 1-2 tablets 3 times daily
- Specific dosages for cystitis, salmonellosis, and urinary tract infections
- Dosage varies for children based on weight and specific infection
- Different dosage for shigellosis in children
Administration
- Tablet should be taken with at least 50-100 ml fluid
- Bioavailability unaffected by simultaneous food intake
- Best taken with or immediately after a meal
Contraindications
- Exclude patients with known allergy to penicillins and cephalosporins
Side Effects
- Generally well tolerated
- May cause upper gastrointestinal disturbances
- Occasional reports of skin rashes
- No evidence of hepato-, nephro-, or ototoxicity
- Potential occurrence of anaphylaxis
Pregnancy & Lactation
- Avoid use in pregnancy due to mecillinam concentrations found in umbilical cord
- Not excreted into the milk of lactating mother
Precautions & Warnings
- Raised blood levels may occur with impaired renal function
Use in Special Populations
- Initial dose unchanged for impaired renal function
- Maintenance dose adjusted based on creatinine clearance
- Avoid use in infants under 3 months of age
Overdose Effects
- Likely to cause nausea, vomiting, and gastritis
- No specific overdose experience with Bacilex
- Treatment restricted to symptomatic and supportive measures
