Pharmacology
- Dihydropyridine calcium-channel blocker
- Long duration of action
- Vasodilation
- Peripheral resistance reduction
- Coronary blood flow increase
- Myocardial oxygen supply increase
- Cardiac output increase
Dosage & Administration
- Usual dose: 5 mg once daily for hypertension
- Maximum dose: 10 mg once daily for hypertension
- Lower dose for elderly and hepatic insufficiency
- 5 to 10 mg for chronic stable or vasospastic angina
- May be taken without regard to meals
Interaction
- Potentially hazardous interactions with p-blockers
- No interaction with Digoxin
- No interaction with Cimetidine
- No significant alteration of Warfarin effect
- Food does not alter absorption
Contraindications
- Hypersensitivity to dihydropyridine derivatives
- Pregnancy

- Eye pain
- Mental depression
- Paradoxical increase in ischaemic chest pain

Pregnancy & Lactation
- Pregnancy Category C
- No adequate and well-controlled studies in pregnant women
- Potential benefit justifies potential risk to fetus
- Recommendation to discontinue nursing

- Hepatic impairment
- Pregnancy

Use in Special Populations
- Special dosage for children with hypertension (6-17 years)
- No known effect on blood pressure in children under 6 years
- Equally well tolerated in elderly
- Recommended dosage in renal impairment
- Dosage recommendation for mild to moderate hepatic impairment
- Not studied in severe hepatic impairment
Overdose Effects
- Excessive peripheral vasodilatation
- Reflex tachycardia
- Marked and prolonged systemic hypotension
- Active cardiovascular support required
- Gastric lavage may be worthwhile
- Use of charcoal within 2 hours of administration
- Dialysis not likely to be beneficial
