Indications
- Essential hypertension
- Angina pectoris
- Vasospastic angina
Pharmacology
- Dihydropyridine calcium-channel blocker
- Vasodilation
- Reduced peripheral resistance
- Increased coronary blood flow
Dosage & Administration
- Usual dose is 5 mg once daily
- Maximum dose is 10 mg once daily
- May be taken without regard to meals
Interaction
- Avoid combination with p-blockers in patients with markedly impaired cardiac left ventricular function
- No interaction with Digoxin
- No interaction with Cimetidine
- No significant alteration of Warfarin effect on prothrombin time
- Food does not alter absorption rate
Contraindications
- Hypersensitivity to dihydropyridine derivatives
- Pregnant woman

- Increased micturition frequency

- Eye pain
- Mental depression
- Rashes
- Fever
- Abnormalities in liver function
Pregnancy & Lactation
- Pregnancy Category C
- Use during pregnancy only if potential benefit justifies potential risk to fetus
Precautions & Warnings
- Precaution in patients with hepatic impairment
- Precaution during pregnancy and breast feeding
Use in Special Populations
- Dosage for children with hypertension (6-17 years)
- Effect on blood pressure in children under 6 years is not known
- Normal dosage recommended in elderly
- Normal dosage recommended in patients with renal impairment
- Caution in dose selection for patients with mild to moderate hepatic impairment
- Initiate at lowest dose for severe hepatic impairment
Overdose Effects
- Excessive peripheral vasodilatation
- Reflex tachycardia
- Systemic hypotension up to and including shock
- Clinically significant hypotension calls for active cardiovascular support
- Vasoconstrictor may be helpful in restoring vascular tone and blood pressure
- Intravenous calcium gluconate may be beneficial
- Gastric lavage may be worthwhile in some cases
- Use of charcoal to reduce absorption rate
- Dialysis not likely to be of benefit
