Indications
- Essential hypertension
- Angina pectoris
- Vasospastic angina
Pharmacology
- Dihydropyridine calcium-channel blocker
- Long duration of action
- Influences myocardial cells
- Vascular smooth muscle vasodilation
Dosage & Administration
- Usual dose of 5 mg once daily for hypertension
- Maximum dose of 10 mg once daily
- 5 to 10 mg for angina
- Lower dose for elderly and hepatic insufficiency
- May be taken without regard to meals
Interaction
- Potentially hazardous interactions with p-blockers
- No interaction with Digoxin
- No interaction with Cimetidine
- No significant alteration with Warfarin
- No alteration by food
Contraindications
- Hypersensitivity to dihydropyridine derivatives
- Pregnancy
Side Effects
- Dizziness
- Flushing
- Headache
- Hypotension
- Peripheral edema
- Gastrointestinal disturbances
- Increased micturition frequency
- Lethargy
- Eye pain
- Mental depression
- Ischaemic chest pain
- Excessive fall in blood pressure
- Rashes
- Fever
- Abnormalities in liver function
Pregnancy & Lactation
- Pregnancy Category C
- Benefit justifies potential risk to fetus
- Discontinue nursing while administered
Precautions & Warnings
- Precaution in patients with hepatic impairment
- Precaution during pregnancy and breast feeding
Use in Special Populations
- Dosage for children with hypertension from 6 to 17 years
- Effect on blood pressure in children under 6 years is unknown
- Elderly patients have similar tolerance
- No correlation between renal impairment and plasma concentrations
- Normal dosage recommended for renal impairment
- Caution in dose selection for hepatic impairment
- Initiate at lowest dose for severe hepatic impairment
Overdose Effects
- Excessive peripheral vasodilatation
- Reflex tachycardia
- Marked and prolonged systemic hypotension
- Active cardiovascular support required
- Elevation of extremities
- Attention to circulating fluid volume and urine output
- Vasoconstrictor may be helpful
- Intravenous calcium gluconate may be beneficial
- Gastric lavage may be worthwhile
- Use of charcoal to reduce absorption rate
- Dialysis not likely to be beneficial
