Indications

  • Treatment of infections caused by Gram-positive and Gram-negative bacteria
  • Upper respiratory tract infections like sinusitis, pharyngitis, tonsillitis, and otitis media
  • Lower respiratory tract infections like bronchitis and pneumonia
  • Urinary tract infections like cystitis, urethritis, and pyelonephritis
  • Skin and soft tissue infections like abscess and cellulitis

Pharmacology

  • Semisynthetic broad spectrum bactericidal antibiotic
  • Active against both gram-positive and gram-negative microorganisms
  • Inhibition of cross-linking process in bacteria cell wall
  • Bactericidal action due to lysis of bacteria

Dosage & Administration

  • Oral Administration - Adults:
    • 500mg four times daily for urinary tract infections
    • 250 to 500mg four times daily for respiratory tract infections
    • 250 to 500mg four times daily for skin and soft tissue infections
  • Oral Administration - Children:
    • Total daily dose of 25 to 50mg/kg for general infections
    • Total daily dose of 75 to 100mg/kg for otitis media
    • Maximum daily dosage: 4 gm
  • Injectable Administration - Adult: 2-4 gm daily in four equally divided doses
  • Injectable Administration - Children: 50-100 mg/kg daily in four equally divided doses

Interaction

Increased risk of kidney damage when used with nephrotoxic drugs

Contraindications

Not to be used in patients with known hypersensitivity to cephalosporins

  • Skin reactions
  • Rare: Glossitis, heartburn, dizziness, nausea, vomiting, diarrhoea, urticaria, joint pains, oedema

Pregnancy & Lactation

  • Safety in pregnancy not established
  • Excreted in breast milk
  • Caution advised in lactating mothers

Precautions & Warnings

  • Prolonged use may result in superinfection
  • Administered with care to patients hypersensitive to penicillins
  • May cause false positive urine glucose result
  • Dosage adjustment necessary in renal impairment
