Indications

  • treatment of upper respiratory tract infections
  • treatment of lower respiratory tract infections
  • treatment of urinary tract infections
  • treatment of skin and soft tissue infections
  • sensitive to Gram-positive bacteria
  • sensitive to Gram-negative bacteria

Pharmacology

  • semisynthetic broad spectrum bactericidal antibiotic
  • active against both gram-positive and gram-negative microorganisms
  • inhibits cross-linking process in cell wall of bacteria
  • lysis of bacteria due to external osmotic pressure
  • main site of action is the cell wall of bacteria

Dosage & Administration

  • 500mg oral dosage for urinary tract infections in adults
  • 250 to 500mg oral dosage for respiratory tract infections in adults
  • 250 to 500mg oral dosage for skin and soft tissue infections in adults
  • 25 to 50mg/kg oral dosage for children
  • 2-4 gm daily injectable dosage for adults
  • 50-100 mg/kg daily injectable dosage for children
  • max daily dosage: 4 gm
  • dosage adjustment necessary in renal impairment

Interaction

  • increased risk of kidney damage with nephrotoxic drugs
  • enhanced renal toxicity with diuretics and probenecid

Contraindications

  • should not be used in patients with known or suspected hypersensitivity to cephalosporins

  • hypersensitivity phenomena
  • skin reactions
  • blood disorders
  • fever
  • jaundice
  • liver enzyme disturbances
  • reversible interstitial nephritis

Pregnancy & Lactation

  • safety in pregnancy not established
  • excreted in breast milk
  • should be used with caution in lactating mothers
  • may cause dizziness

Precautions & Warnings

  • prolonged use may result in superinfection
  • administered with care to patients hypersensitive to penicillins
  • may cause false positive urine glucose result
  • dosage adjustment necessary in renal impairment
  • contains lactose

Use in Special Populations

  • recommended doses for patients with renal impairment or on haemodialysis
  • higher doses may be required for chronic or severe infections
  • regular bacteriological and clinical monitoring required for urinary tract infections
  • paediatric dosages should not exceed those specified for adults

Overdose Effects

  • non-specific symptoms like nausea, vomiting, and diarrhoea
  • treatment is mainly supportive

Therapeutic Class

  • First generation Cephalosporins

Storage Conditions

  • reconstituted Suspension should be used within 7-14 days
  • injection solutions should be used within 2 hours
  • retain potency for 12 hours when stored at 5°C
  • do not use later than the date of expiry
