Therapeutic Class

  • First generation Cephalosporins

Indications

  • Treatment of infections caused by Gram-positive and Gram-negative bacteria
  • Upper respiratory tract infections like sinusitis, pharyngitis, laryngo-tracheo bronchitis
  • Lower respiratory tract infections like bronchitis, lobar pneumonia, bronchopneumonia
  • Urinary tract infections like cystitis, urethritis, pyelonephritis
  • Skin and soft tissue infections like abscess, cellulitis, furunculosis, impetigo

Pharmacology

  • Semisynthetic broad spectrum bactericidal antibiotic
  • Active against both gram-positive and gram-negative microorganisms
  • Inhibits cross-linking process in the cell wall of bacteria, leading to lysis

Dosage & Administration

  • For oral administration in adults and children
  • For injectable administration in adults and children
  • Dosage adjustment necessary in renal impairment
  • Maximum daily dosage: 4 gm
  • Monitor patients with impaired renal or hepatic function

Interaction

  • Concomitant use with nephrotoxic drugs may increase risk of kidney damage
  • Diuretics and probenecid enhance possibility of renal toxicity

Contraindications

  • Hypersensitivity to cephalosporins

  • Skin reactions
  • Blood disorders, immune system disorders, psychiatric disorders

Pregnancy & Lactation

  • No established safety in pregnancy
  • Use with caution in lactating mothers

Precautions & Warnings

  • Prolonged use may result in superinfection
  • Administer with care to patients hypersensitive to penicillins
  • May cause false positive urine glucose result

Use in Special Populations

  • Recommended doses for renal impairment and haemodialysis
  • Higher doses may be required for chronic or severe infections
  • Regular bacteriological and clinical monitoring required for chronic urinary tract infections

Overdose Effects

  • Symptoms include nausea, vomiting, diarrhoea, gastric upsets
  • Treatment mainly supportive
  • Gastric lavage may be necessary if large amount ingested

Storage Conditions

  • Suspension should be freshly prepared
  • Reconstituted Suspension should be used within 7-14 days
  • Injection solutions should be used within 2 hours at room temperature
  • Reconstituted solutions retain potency for 12 hours when stored at 5°C
  • Keep out of reach of children
  • To be dispensed only on prescription
